*This process only applies to alumni requesting transcripts, current Euclid High School students should reach out to their School Counselor.
How can I obtain a copy of my transcript or records?
Requesting a transcript or records online
Click this link to complete the required information, and to also pay your fees ($5.00 for transcripts, $0.10 per page for other records). You will have the option of having the transcript or records mailed to you, or picking them up in person at our main office at Euclid High School (711 E. 222nd St., Euclid OH 44123).
Requesting a transcript or records in PERSON:
Print this request form and complete the required information.
Call Registration at 216-797-2932 or Euclid High School’s Guidance Department (during the school year) at 216-797-7814 to schedule an appointment. Proper photo identification must be presented.
The fee for transcripts is $5.00, payable with cash or money order (no personal checks).
The fee for records is $0.10 per copied page, payable with cash or money order (no personal checks).
Requesting a transcript by U.S. MAIL:
Print this request form and complete the required information.
Enclose a copy of your state ID or driver’s license.
Include the fee of $5.00 cash or money order (payable to Euclid City Schools).

Upon receipt of your request, please allow 24 hours to process all requests. Please note that outstanding student fees must be paid before your transcript will be issued. The fees may be paid in the Treasurer’s Office at the Euclid Board of Education.
