School Clinics
Each school clinic is staffed throughout the school day by a health monitor. Our goal is to keep every student healthy, active, and ready to learn. If your student requires medication during the school day or has a health condition that will require special care during the upcoming school year, please check our resource links, download and print any applicable forms. Once forms are completed by yourself and your student’s physician, you may fax, mail or walk forms into your student’s school clinic.
